    Background:

    • De Quervain's thyroiditis is an inflammatory condition affecting the thyroid gland.

    Observation:

    • A 53-year-old woman presented with early de Quervain's disease affecting both thyroid lobes.
    • Iodine uptake in the thyroid was suppressed, despite normal serum thyroid hormone levels.
    • Thyroid hormone levels increased significantly after thyrotropin-releasing hormone (TRH) administration, indicating normal pituitary response.
    • Thyroid reserve was found to be low after thyroid-stimulating hormone (TSH) administration.

    Findings:

    • Biopsy confirmed sub-acute thyroiditis.
    • The suppressed iodine uptake in early de Quervain's thyroiditis appears to be a direct result of glandular inflammation.
    • This contrasts with typical scenarios where suppressed uptake is due to elevated thyroid hormones suppressing TSH secretion.

    Implications:

    • The findings suggest a novel mechanism for thyroid dysfunction in early de Quervain's thyroiditis.
    • This highlights the importance of considering inflammatory processes as a primary cause of altered thyroid function.
    • Early diagnosis and understanding of these mechanisms can guide appropriate patient management.
