Peripapillary atrophy in primary angle-closure glaucoma differs from primary open-angle glaucoma. These findings suggest distinct mechanisms in optic disk damage development for each glaucoma type.

Background:

  • Peripapillary atrophy (PPA) is a common finding in glaucoma.
  • Understanding PPA characteristics is crucial for diagnosing and managing different glaucoma subtypes.

Purpose of the Study:

  • To characterize peripapillary atrophy (PPA) in primary angle-closure glaucoma (PACG).
  • To compare PPA in PACG with that observed in primary open-angle glaucoma (POAG).

Main Methods:

  • Eyes with PACG (n=21) and POAG (n=31) were analyzed, matched for cup-to-disk ratio and visual field mean deviation.
  • Prevalence and ratio of PPA to disk area were compared between PACG and POAG groups.
  • Correlation between PPA area and optic disk/visual field parameters was assessed.

  • PPA was present in 38% of PACG eyes versus 68% of POAG eyes (P = .048).
  • The PPA-to-disk area ratio was significantly lower in PACG (0.16) compared to POAG (0.41) (P = .005).

Conclusions:

  • Peripapillary atrophy in PACG exhibits a different relationship with structural and functional optic disk changes compared to POAG.
  • Distinct pathophysiological mechanisms likely underlie optic disk damage in PACG and POAG.
