[The immunologic status during pulmonry tubercuolsis].

A Genoud, J J Farquet, E Arnold

    Schweizerische Medizinische Wochenschrift

    Summary

This study evaluated immune responses in tuberculosis patients, finding frequent anergy, especially in older adults. Impaired cellular immunity was linked to negative skin tests and specific clinical factors in patients with pulmonary tuberculosis.

    Area of Science:

    • Immunology
    • Infectious Diseases
    • Pulmonology

    Background:

    • Pulmonary tuberculosis (TB) significantly impacts immune function.
    • Assessing cellular and humoral immunity is crucial for understanding TB pathogenesis and patient outcomes.
    • Previous studies indicate immune dysregulation in TB patients, but comprehensive evaluation is ongoing.

    Purpose of the Study:

    • To prospectively evaluate cellular and humoral immunity in patients with pulmonary tuberculosis.
    • To correlate immune responses with clinical parameters and PPD skin test results.
    • To identify factors associated with immune defects in TB patients.

    Main Methods:

    • Prospective evaluation of 22 PPD-positive and 10 PPD-negative TB patients.
    • Immune assessments included intradermoreaction (IDR), lymphocyte blast transformation (SL), MIF production, peripheral lymphocyte count, and immunoglobulin levels.
    • Stimulants used were PPD, Candida, and varidase.

    Main Results:

    • A strong correlation was observed between different immunological tests.
    • Anergy was frequently detected, particularly in elderly patients.
    • Negative immune responses (IDR, SL, MIF) were significantly associated with negative PPD-IDR, lymphopenia (<1,000/mm³), impaired PHA-induced blast transformation (<21,000 cpm), and cavitary TB.

    Conclusions:

    • Patients with pulmonary tuberculosis frequently exhibit defects in cellular immunity.
    • Immune anergy is a notable finding, especially in elderly TB patients.
    • Specific clinical and immunological markers can indicate a compromised cellular immune system in TB.
