Summary

Precut sphincterotomy is a safe and effective endoscopic retrograde cholangiopancreatography (ERCP) technique for difficult common bile duct cannulation. This method significantly increases successful cannulation rates in challenging cases.

Area of Science:

  • Gastroenterology
  • Endoscopic Procedures
  • Biliary Tract Interventions

Background:

  • Precut sphincterotomy has faced safety concerns.
  • Tertiary centers report success in increasing common bile duct cannulation rates with this technique.

Purpose of the Study:

  • To evaluate the efficacy and safety of precut sphincterotomy.
  • To correlate complication rates with bile duct diameter.

Main Methods:

  • Precut sphincterotomy was performed on 120 patients with failed standard cannulation attempts.
  • Success rates and complication rates were analyzed.
  • Bile duct diameters were correlated with complications.

Main Results:

  • Successful common bile duct cannulation and sphincterotomy achieved in 96% of patients.

Related Experiment Videos

  • Complications occurred in 6.9% of patients (bleeding, retroduodenal perforation).
  • No procedure-related mortality; complications managed medically or surgically in one case.
  • Conclusions:

    • Precut sphincterotomy is effective and safe for difficult common bile duct cannulation.
    • Excluding patients with small bile duct sizes may reduce complication rates.
