Surgical endoscopy·2024

Area of Science:

  • Minimally Invasive Surgery
  • Gastrointestinal Surgery
  • Surgical Techniques

Background:

  • Laparoscopic cholecystectomy is a common procedure for gallbladder removal.
  • Standard laparoscopic cholecystectomy typically involves four abdominal ports.
  • Alternative techniques aim to reduce invasiveness and improve patient outcomes.

Purpose of the Study:

  • To compare the outcomes of transumbilical cholecystectomy with standard laparoscopic cholecystectomy.
  • To evaluate differences in pain, analgesia requirements, cost, side effects, and cosmesis.

Main Methods:

  • A randomized open study was conducted at a teaching hospital in Italy.
  • Ninety patients undergoing elective cholecystectomy were included.
  • Procedures compared were standard 4-port vs. transumbilical 2-port laparoscopic cholecystectomy.

Related Experiment Videos

Main Results:

  • No complications or conversions to open surgery were reported in either group.
  • Transumbilical cholecystectomy resulted in significantly lower pain scores (p<0.05).
  • Patients undergoing transumbilical cholecystectomy required significantly less analgesia in the first 24 hours (p<0.05).

Conclusions:

  • Transumbilical cholecystectomy is a feasible alternative to standard laparoscopic cholecystectomy.
  • This technique can be performed effectively once the surgeon has completed the learning curve.
  • The transumbilical approach may offer advantages in terms of patient comfort and recovery.
