Carbon monoxide (CO) poisoning can cause toxic optic neuropathy, affecting visual function. Early treatment with hydroxycobalamine may improve outcomes in patients with CO poisoning.

Background:

  • Carbon monoxide (CO) poisoning is a significant public health concern.
  • Visual system dysfunction can be a consequence of CO poisoning.

Observation:

  • This study investigated the electrophysiological and psychophysical effects of CO poisoning on visual function in three patients.
  • Ocular examinations, Goldmann visual fields, flash and pattern visual evoked potentials (VEPs), and flash and pattern electroretinograms (ERGs) were performed.

Findings:

  • Electrophysiological testing revealed reduced or absent N95 components in pattern ERGs and delayed, reduced VEPs.
  • A positive-negative-positive (PNP) VEP waveform was observed in two patients.
  • One patient showed improvement in visual and electrophysiological function with early hydroxycobalamine treatment.

Implications:

  • CO poisoning can lead to toxic optic neuropathy with potential similarities to tobacco amblyopia.
  • Early intervention with hydroxycobalamine may offer therapeutic benefits for visual disturbances caused by CO poisoning.
