Cardiac surgery in patients aged 80 and older can achieve low operative mortality and excellent quality of life. However, emergency and mitral valve procedures in this elderly population yield less favorable outcomes.

Area of Science:

  • Cardiology
  • Geriatric Medicine
  • Surgical Outcomes

Background:

  • Increasing number of elderly patients undergoing cardiac surgery.
  • Elderly patients are often symptomatic and at high risk for intervention.

Purpose of the Study:

  • To review the experience with cardiac surgery in patients aged 80 years and older.
  • To assess outcomes, including mortality, quality of life, and survival rates.

Main Methods:

  • Retrospective review of 242 patients aged 80+ undergoing cardiopulmonary bypass surgery (1981-1997).
  • Analysis of pre-operative conditions, surgical procedures, and post-operative outcomes.
  • Statistical analysis including Cox's proportional hazards model.

Main Results:

Related Experiment Videos

  • Early mortality was 5.7%, with mitral valve procedures and emergency surgery associated with increased risk.
  • Median ITU and in-hospital stay were 1 and 10 days, respectively.
  • 93% of patients lived independently at 2 months; 5-year survival was 67.7%, with isolated aortic valve replacement (AVR) and coronary artery bypass grafting (CABG) showing better survival than mitral surgery.

Conclusions:

  • Cardiac surgery is feasible in selected elderly patients with acceptable operative mortality.
  • Elderly patients can achieve an excellent quality of life and survival post-operatively.
  • Mitral valve surgery and emergency procedures in this demographic are associated with poorer outcomes.
