Summary

Hormone replacement therapy may be safe for women treated for colorectal cancer. Studies suggest it can be beneficial without worsening prognosis, especially after palliative care or relapse.

Area of Science:

  • Oncology
  • Gynecology
  • Endocrinology

Background:

  • Colorectal carcinoma is a prevalent malignancy in Western countries.
  • Gynecologists frequently encounter questions regarding hormone replacement therapy (HRT) post-colorectal cancer surgery.

Purpose of the Study:

  • To evaluate the safety and potential benefits of sexual steroid hormone replacement therapy in women after curative treatment for colorectal cancer.
  • To determine if HRT is justifiable in this patient population, analogous to breast cancer treatment.

Main Methods:

  • An observational study involving 52 women (42 colon, 10 rectal cancer) treated with a combined hormone preparation between 1989 and 1997.
  • Analysis based on data from international publications and own study findings.

Main Results:

  • Estrogen and progesterone receptors are typically absent or present in low concentrations in colorectal cancer cells.
  • HRT is considered justifiable in receptor-negative breast cancer patients, suggesting a similar approach for colorectal cancer patients.
  • Withholding HRT should not be done without evidence of worsening prognosis.

Conclusions:

  • Combined hormone replacement therapy can be beneficial for symptom management after palliative treatment or in cases of relapse.
  • HRT should be recommended following a thorough risk-benefit assessment and patient consultation.
