Summary

Environmental control measures significantly improve atopic eczema symptoms in patients allergic to house-dust mites (HDM). Active treatments, including allergen-proof bedding, were superior to placebo in reducing eczema severity.

Area of Science:

  • Allergen immunotherapy
  • Dermatology
  • Environmental health

Background:

  • House-dust mite (HDM) allergy is a common trigger for atopic eczema.
  • Prick and patch testing can detect allergic reactivity to HDM in affected patients.
  • The clinical relevance of HDM allergy in atopic eczema requires further investigation.

Purpose of the Study:

  • To evaluate the clinical efficacy of anti-HDM environmental control measures in patients with atopic eczema.
  • To assess the impact of active treatment versus placebo on eczema severity and allergen levels.

Main Methods:

  • A 6-month, placebo-controlled, double-blind trial involving 48 patients with atopic eczema.
  • Active treatment included Gortex bedding encasements, a high-powered vacuum, and an acaricide spray.
  • Placebo treatment involved cotton bags, a basic vacuum, and a water spray.
  • Dust samples (Der p 1 allergen) and clinical eczema scores were assessed regularly by blinded observers.

Main Results:

  • Active treatment with Gortex bed bags significantly reduced dust and Der p 1 allergen within bedding.
  • Both active and placebo treatments reduced carpet Der p 1 levels to minimal concentrations.
  • Patients receiving active treatment showed significantly greater improvement in eczema scores compared to the placebo group (P<0.0006).

Conclusions:

  • Gortex bed bags are highly effective in containing dust allergens within bedding.
  • Environmental control measures, particularly allergen-impermeable bedding, significantly improve atopic eczema symptoms.
  • The greatest clinical benefits were observed in patients with the most severe eczema.
