    Summary

    Isolated Bence-Jones proteinuria was detected in two diabetic patients without evidence of cancer. These cases suggest benign proteinuria can occur independently of malignant B-cell disorders.

    Background:

    • Diabetic patients undergo screening for proteinuria.
    • Bence-Jones proteinuria is typically associated with multiple myeloma or other B-cell malignancies.

    Observation:

    • Two diabetic patients presented with isolated Bence-Jones proteinuria during routine screening.
    • One patient had plasmocytosis but no other signs of myelomatosis.
    • The second patient showed no clinical or laboratory evidence of myelomatosis.

    Findings:

    • The cases suggest isolated Bence-Jones proteinuria can occur without malignant B-cell dyscrasia.
    • These instances may represent benign or idiopathic Bence-Jones proteinuria.

    Implications:

    • This challenges the assumption that Bence-Jones proteinuria always indicates malignancy.
    • Further long-term follow-up is needed to definitively classify these cases.
    • Highlights the importance of comprehensive evaluation in diabetic patients with proteinuria.
