Hospital-based discharge planning: the beginning of home care for many

Discharge planning can be viewed as the first step in home care for many individuals. This article examines the process through the discussion of the stages of discharge planning, and a study of discharge planning in an acute-care hospital. Several recommendations are made that would strengthen the role of home care providers in the process, who are encouraged to work for a more active role in discharge planning.
