    • A 19-year-old male with a solitary kidney developed a large Douglas' abscess.
    • The patient subsequently experienced anuria (cessation of urine production).

    Findings:

    • The anuria was likely caused by ureteral obstruction due to the abscess.
    • A proposed mechanism is longitudinal tension on the ureter over the iliac vessels, termed the 'iliac sign'.

    Implications:

    • This case highlights the critical risk of ureteral obstruction in patients with single kidneys experiencing abdominal abscesses.
    • Early recognition of the 'iliac sign' may be crucial for timely intervention and preventing renal failure.
