Abnormalities in gallbladder morphology and function in patients with cholelithiasis.

D L Nahrwold, R C Rose, S P Ward

    Annals of Surgery
    |October 1, 1976
    PubMed
    Summary

Clinical symptoms do not reliably predict gallbladder histology or function in patients with gallstones. Gallbladder absorptive capability correlates with histologic abnormalities, not clinical presentation.

    Area of Science:

    • Gastroenterology
    • Surgical Pathology
    • Physiology

    Background:

    • Cholelithiasis (gallstones) is a common condition often treated with cholecystectomy.
    • Understanding the relationship between clinical symptoms, gallbladder histology, and function is crucial for patient management.

    Purpose of the Study:

    • To investigate the correlation between clinical manifestations, histologic findings, and gallbladder absorptive capability in patients undergoing cholecystectomy for symptomatic cholelithiasis.
    • To assess the predictive value of clinical and histologic scoring systems for gallbladder function.

    Main Methods:

    • A cohort of 37 patients with symptomatic cholelithiasis undergoing cholecystectomy was studied.
    • Clinical data were used to derive a clinical score.
    • Histologic parameters indicative of gallbladder disease were used to calculate a histologic score.

    Related Experiment Videos

  • Short-circuit current measurements were employed to quantify gallbladder sodium absorption, reflecting absorptive function.
  • Main Results:

    • A high clinical score did not reliably predict abnormal gallbladder histology or reduced absorptive function.
    • Gallbladder absorptive capability demonstrated a direct relationship with the degree of histologic abnormality.
    • Gallbladder absorptive capability was found to be a continuous phenomenon, not an all-or-none state.
    • Oral cholecystography visualization was found to be an unreliable indicator of gallbladder absorptive capability.

    Conclusions:

    • Clinical presentation is a poor predictor of gallbladder histologic status and absorptive function.
    • Gallbladder absorptive function is intrinsically linked to the severity of histologic changes.
    • Oral cholecystography is not a dependable method for assessing gallbladder absorptive capacity.
