Abstract:
Health care is undergoing a quiet revolution in communities across America as purchasers, providers and consumers explore new options for controlling health care costs, expanding access and improving health status. A major Michigan effort in this regard is the Comprehensive Community Health Models (CCHMs) Initiative funded by the W.K. Kellogg Foundation.
