Plasma exchange may benefit certain Chronic Inflammatory Demyelinating Polyneuropathies (CIDP) patients, particularly those with paraproteinemias or autoimmune markers. Effectiveness varied, with best responses seen in paraproteinemic CIDP cases.

Background:

  • Chronic Inflammatory Demyelinating Polyneuropathies (CIDP) involve peripheral nerve demyelination and are suspected to have an immune-mediated pathogenesis.
  • Current treatments like immunosuppressants, plasmapheresis, and intravenous immunoglobulins show variable success rates.

Purpose of the Study:

  • To review plasma exchange outcomes in 20 CIDP patients with diverse underlying conditions.
  • To identify predictors for plasma exchange effectiveness in CIDP management.

Main Methods:

  • Retrospective analysis of 20 CIDP patients treated with plasma exchange (4-31 plasma volumes, median 12, with 5% albumin).
  • Patients included those with monoclonal gammopathies, various cancers, hemolytic anemia, and idiopathic CIDP (some with autoimmune markers).

  • Overall, 2 excellent, 1 very good, and 2 moderate transient responses were observed.
  • 12 patients showed no change in clinical or electrophysiological status.

  • Plasma exchange may be a viable immune modulation strategy for CIDP patients presenting with paraproteinemias.
  • CIDP patients with autoimmune manifestations might also benefit from plasma exchange therapy.
