Summary

Generalized spike-wave (S-W) on electroencephalograms (EEG) is rare in children. Most children with S-W are epileptic, but a small percentage with febrile seizures are not, avoiding future epilepsy.

Area of Science:

  • Pediatric Neurology
  • Clinical Neurophysiology

Context:

  • Electroencephalograms (EEG) are crucial for diagnosing neurological disorders in children.
  • Generalized spike-wave (S-W) patterns on EEG can indicate epilepsy but require careful interpretation.

Purpose:

  • To assess the clinical significance of generalized spike-wave (S-W) patterns on pediatric electroencephalograms (EEG).
  • To specifically evaluate the relevance of S-W patterns in children experiencing febrile seizures.

Summary:

  • This study analyzed 39,322 EEGs, identifying 170 pediatric patients with S-W patterns.
  • Of these, 154 (90.6%) were diagnosed with epilepsy, while 16 (9.4%) were not.
  • Notably, 12 of the 16 non-epileptic children had a history of febrile seizures and did not develop epilepsy during a 4.5-year follow-up.

Impact:

  • The findings suggest that S-W patterns on pediatric EEG are uncommon.
  • A small proportion of children with S-W, particularly those with febrile seizures, may not have epilepsy, reducing diagnostic confusion.
  • This highlights the importance of considering clinical history alongside EEG findings in pediatric neurological assessments.
