Area of Science:

  • Gastroenterology
  • Surgical Innovation
  • Biliary System Research

Background:

  • Routine biliary drainage after common bile duct exploration (CBDE) is standard practice.
  • Concerns exist regarding the safety and necessity of routine drainage.
  • Primary closure of the common bile duct offers a potential alternative.

Purpose of the Study:

  • To evaluate the safety and efficacy of primary closure of the common bile duct.
  • To compare outcomes of primary closure versus routine drainage in selected patients.

Main Methods:

  • Twenty patients undergoing CBDE had primary closure of the common bile duct.
  • Data collected included liver function tests, imaging (ultrasound, cholangiography), operation time, complications, and hospital stay.

Related Experiment Videos

Main Results:

  • Nineteen of twenty patients experienced no biliary complications.
  • Minor complications included bile leakage (1) and duodenal leakage (1).
  • Liver function tests normalized within 2-3 weeks; postoperative imaging showed no stenosis or residual stones.

Conclusions:

  • Primary common bile duct closure is a safe and viable option for selected patients.
  • It can be an alternative to routine biliary drainage, potentially reducing morbidity.
