Background:

  • Behcet's disease (BD) can affect the central nervous system (CNS), with reported prevalence rates ranging from 4-49%.
  • CNS involvement typically manifests months to years after systemic symptoms, but can be the initial presentation in 5% of cases.
  • Variability in prevalence suggests potential subclinical neurologic involvement in BD patients.

Purpose of the Study:

  • To investigate subclinical neurologic involvement in Behcet's disease patients without neurological symptoms.
  • To explore the utility of cerebral single-photon emission computed tomography (SPECT) in detecting early-stage CNS abnormalities in BD.

Main Methods:

  • 20 Behcet's disease patients without neurological symptoms underwent cerebral SPECT.
  • A control group with non-neurologic conditions was included for comparison.
  • Brain MRI was performed for patients with abnormal SPECT findings.

Main Results:

  • Cerebral SPECT revealed decreased and asymmetrical tracer uptake in 35% of Behcet's disease patients.
  • Brain MRI scans were normal in these patients, despite abnormal SPECT findings.
  • Functional imaging detected abnormalities before morphological changes were evident on MRI.

Conclusions:

  • Cerebral SPECT can identify subclinical neurologic involvement in Behcet's disease patients.
  • SPECT may detect early-stage functional abnormalities in the CNS before structural damage occurs.
  • This imaging technique is valuable for diagnosing early neurologic changes in BD, even in asymptomatic individuals.
