Summary

Roth spots, or white-centered retinal hemorrhages, were once thought to indicate bacterial endocarditis. However, recent evidence shows these spots are nonspecific and appear in various diseases, requiring a broader differential diagnosis.

Area of Science:

  • Ophthalmology
  • Pathology
  • Infectious Diseases

Background:

  • Roth spots (white-centered retinal hemorrhages) are classically associated with subacute bacterial endocarditis.
  • Historically, they were considered pathognomonic for this specific cardiac infection.

Observation:

  • Recent histological studies challenge the traditional view of Roth spots.
  • Evidence suggests they are not exclusively indicative of bacterial abscesses in the retina.

Findings:

  • Roth spots are nonspecific findings.
  • They can manifest in a wide range of medical conditions beyond bacterial endocarditis.

Implications:

  • The pathogenesis of white-centered hemorrhages requires re-evaluation.
  • Clinical work-up for Roth spots must include a broad differential diagnosis.
  • Rethinking the diagnostic significance of Roth spots in clinical practice is necessary.
