    Area of Science:

    • International Health
    • Maternal and Child Health
    • Nutrition and Infectious Disease Interactions

    Background:

    • High prevalence of malnutrition, undernutrition, and infectious diseases in preschool children in rural North India.
    • Limited accessibility and underutilization of existing governmental health services.
    • Need to evaluate integrated health interventions for improving child health outcomes.

    Purpose of the Study:

    • To investigate the effects of combined nutrition and medical care on child growth, morbidity, and mortality.
    • To assess the cost-effectiveness of different health intervention strategies.
    • To evaluate the role of auxiliary health workers in delivering primary healthcare.

    Main Methods:

    • Longitudinal and cross-sectional surveys in 10 villages of Punjab, North India, from 1968-1973.
    • Allocation of villages into control and service groups (nutrition care, medical care, or both).
    • Monitoring of service inputs, costs, and health outcomes including growth, hemoglobin levels, and mortality rates.

    Main Results:

    • Significant improvements in child growth (weight, height) and hemoglobin levels.
    • Reduced perinatal mortality through maternal nutrition supplementation.
    • Reduced postneonatal and child mortality, and decreased illness duration with medical care.
    • Auxiliary health workers managed over 90% of health needs effectively.

    Conclusions:

    • Integrated nutrition and medical care interventions are highly effective in improving child health outcomes in developing countries.
    • Prenatal nutrition and early childhood medical care are the most cost-effective interventions for reducing child mortality.
    • The study highlights the critical role of primary healthcare workers and integrated service delivery models.
