Summary

A rare case of a giant left atrial appendage compressing the lung was successfully treated. Mitral valve replacement and left atrial plication were performed via the enlarged appendage.

Background:

  • Giant left atrial appendage (LAA) is a rare condition.
  • Chronic massive mitral regurgitation can lead to significant cardiac and pulmonary complications.

Observation:

  • A 45-year-old male presented with a giant left atrial appendage (13x10 cm).
  • The massive LAA compressed the left lower lobe and displaced the mediastinum.
  • The patient also had chronic massive mitral regurgitation.

Findings:

  • Successful chordal sparing mitral valve replacement was performed.
  • Left atrial plication was achieved through the giant LAA.
  • The surgical approach utilized median sternotomy.
