Linear lichen planus (LP) is a rare skin condition presenting as linear papules. This case highlights a unique presentation of unilateral linear LP, later developing into typical oral lichen planus.

Background:

  • Lichen planus (LP) is an inflammatory condition affecting skin and mucous membranes.

Observation:

  • A 33-year-old woman presented with a 4-month history of pruritic, unilateral, linear papular lesions on her neck.
  • These neck lesions were clinically and histologically consistent with linear LP.
  • Subsequently, she developed typical oral LP with white, lacy lesions on the left side of her tongue and buccal mucosa.

Findings:

  • The patient's presentation of unilateral linear papules on the neck mimicked lichen striatus or lichenoid epidermal nevus.
  • Histological examination and the development of oral LP confirmed the diagnosis of linear LP with unilateral restriction.
