Delayed interval delivery in high-risk multiple gestations shows promise for improving newborn survival. While not yet statistically significant, this approach warrants further study to optimize outcomes for premature infants.

    Area of Science:

    • Perinatology
    • Maternal-Fetal Medicine
    • Reproductive Endocrinology

    Background:

    • Increasing incidence of higher-order gestations due to assisted reproductive technologies.
    • Associated rise in maternal and fetal complications, including high perinatal mortality rates.
    • Critical window for perinatal mortality and morbidity is 23-28 weeks of gestation.

    Observation:

    • Two case reports of delayed interval delivery for twin and triplet gestations, both delivered at 21 weeks with a 5-day interval.
    • Neither case involved monochorionic/monoamniotic gestations.
    • Outcomes included extreme prematurity for twins and stillbirth/extreme prematurity for triplets.

    Findings:

    • Literature review reports 48 twin pregnancies with delayed interval delivery, resulting in 40 surviving infants (96 fetuses).

    Related Experiment Videos

  • Delaying delivery of remaining fetuses appears to improve their prognosis.
  • No current consensus on technique or statistically significant evidence supporting specific methods.
  • Implications:

    • Delayed interval delivery may improve survival and decrease morbidity in extremely preterm multiple gestations.
    • Further research is indicated to establish optimal techniques and demonstrate statistical significance.
    • Reducing preterm birth in multiple gestations is crucial for improving neonatal outcomes and decreasing healthcare costs.
