Financing graduate medical education and biomedical research: whose responsibility?

In the United States, biomedical research and graduate medical education are particularly vulnerable in the current era of prospective payment and price competition. Traditionally these services have been financed through the cross-subsidization of patient care dollars, and this practice cannot continue in the current economic environment. This paper reviews the various options for financing these services and makes several specific recommendations.
