Collecting duct carcinoma, a rare kidney cancer, can present with aggressive sarcomatoid features. This aggressive tumor type has a poor prognosis, highlighting the need for further research.

Background:

  • Collecting duct carcinoma (CDC) is a rare subtype of renal cell carcinoma.
  • CDC exhibits aggressive clinical behavior and distinct pathological features.

Observation:

  • A case of a fatal collecting duct carcinoma with sarcomatoid features in an 80-year-old male is presented.
  • The tumor displayed both adenocarcinomatous and sarcomatous components with infiltrating margins.
  • Immunohistochemistry revealed positivity for cytokeratin 19 and specific lectins in the adenocarcinomatous areas.

Findings:

  • The sarcomatous component resembled malignant fibrous histocytoma.
  • Flow cytometry indicated the tumor was DNA diploid.
  • The patient succumbed to the disease within 3 months of diagnosis.

Implications:

  • Sarcomatoid collecting duct carcinoma represents a highly aggressive renal tumor.
  • This case underscores the poor prognosis associated with this rare cancer subtype.
  • Further investigation into CDC pathogenesis and treatment is warranted.
