Summary

A rare complication of laparoscopic colonic surgery, transmesenteric incarcerated hernia, can occur. Complete mesenteric closure after bowel resection is crucial to prevent this serious postoperative obstruction.

Area of Science:

  • Minimally Invasive Surgery
  • Gastrointestinal Surgery

Background:

  • Laparoscopic-assisted surgery is increasingly utilized for various colonic procedures.
  • Potential complications, though rare, require careful consideration and prevention strategies.

Observation:

  • A 47-year-old female patient developed bowel obstruction 20 days post-laparoscopic-assisted sigmoid colectomy.
  • Conservative management with a long tube decompression was unsuccessful.
  • Surgical exploration revealed small bowel incarceration within a mesenteric defect from the prior surgery.

Findings:

  • Transmesenteric incarcerated hernia is a preventable complication following laparoscopic bowel resection.
  • Complete closure of mesenteric defects is essential to avert internal hernias.

Related Experiment Videos

Implications:

  • Surgeons performing laparoscopic colonic surgery must prioritize meticulous mesenteric defect closure.
  • This practice can significantly reduce the risk of postoperative bowel obstruction and associated morbidity.
