Summary

Polyradiculopathy, a rare complication of sarcoidosis, often affects the lower body and may improve with corticosteroid treatment. This condition involves nerve roots and should be considered in sarcoidosis patients experiencing weakness.

Area of Science:

  • Neurology
  • Immunology
  • Rheumatology

Background:

  • Sarcoidosis is a multisystem inflammatory disease.
  • Polyradiculopathy is a rare neurological complication of sarcoidosis.

Observation:

  • This study reviewed 17 cases (3 new, 14 retrospective) of polyradiculopathy in sarcoidosis.
  • Common symptoms included lower extremity weakness (71%), areflexia (59%), and sphincter dysfunction (35%).
  • Cases frequently co-occurred with central nervous system sarcoidosis and primarily involved thoracolumbar or lumbosacral nerve roots.

Findings:

  • Polyradiculopathy in sarcoidosis is uncommon, typically affecting thoracic and lumbar roots.
  • Potential mechanisms include contiguous, hematogenous, or gravitational seeding of nerve root sleeves.

Related Experiment Videos

  • The condition can be asymptomatic and may show improvement with corticosteroid therapy or laminectomy.
  • Implications:

    • Nerve root involvement should be considered in the differential diagnosis of weakness in sarcoidosis patients.
    • Early recognition and treatment may improve outcomes for sarcoidosis-associated polyradiculopathy.
    • Further research into the pathogenesis and optimal management strategies is warranted.
