Summary

Elevated homocysteine levels, linked to vitamin deficiencies and smoking, may signal atherosclerosis. Prompt diagnosis and vitamin treatment are crucial for women with adverse pregnancy outcomes and cerebrovascular risks.

Area of Science:

  • Cardiovascular Medicine
  • Obstetrics & Gynecology
  • Nutritional Science

Background:

  • Atherosclerosis, a condition of arterial hardening, can manifest subtly during pregnancy.
  • Hyperhomocysteinemia, elevated homocysteine levels, is an emerging risk factor for vascular disease.
  • Dietary vitamin deficiencies and smoking exacerbate hyperhomocysteinemia.

Observation:

  • Two case reports highlight the link between obstetrical complications and atherosclerosis.
  • Patients presented with hyperhomocysteinemia, vitamin deficiencies, and smoking history.
  • Adverse pregnancy outcomes included intrauterine fetal death, placental infarction, abortion, and fetal dysmaturity.

Findings:

  • Both patients experienced cerebrovascular accidents, with evidence of prior cerebral infarctions.
  • Hyperhomocysteinemia, even mild or genetically predisposed, appears linked to severe vascular events.
  • Risk factors included smoking, poor diet, and specific drug interactions affecting vitamin levels.

Implications:

  • Women with recurrent abortions, pre-eclampsia, or fetal growth issues warrant screening for hyperhomocysteinemia.
  • Vitamin supplementation (folic acid, B6, B12) may be a preventive strategy.
  • Further research is needed to confirm the efficacy of vitamin therapy in preventing atherosclerotic manifestations.
