Background:

  • Hairy cell leukemia (HCL) is a rare B-cell malignancy.
  • Postoperative chylothorax is a rare but serious complication following thoracic or neck surgery.
  • Managing chylothorax in patients with hematologic malignancies presents unique challenges.

Observation:

  • A 43-year-old black woman with a history of hairy cell leukemia developed left chylothorax postoperatively.
  • Initial management with pleural drainage provided temporary relief but was insufficient for definitive treatment.

Findings:

  • A combination of chemotherapy and an elemental enteral diet was administered.
  • The diet was specifically enriched with medium-chain triglycerides (MCTs) to facilitate lymphatic absorption.
  • This multimodal approach led to successful resolution of the chylothorax.

Implications:

  • This case highlights a potential therapeutic strategy for managing chylothorax in immunocompromised patients.
  • The use of MCT-enriched enteral nutrition may be beneficial in similar clinical scenarios.
  • Further research could explore the efficacy of this combined approach in a larger cohort.
