Summary

A rare fungal eye infection, mycotic keratitis, was caused by an unusual strain of Exserohilum rostratum in a patient with Hansens disease. This case highlights the importance of considering atypical pathogens in diagnosing fungal keratitis.

Area of Science:

  • Ophthalmology
  • Medical Mycology
  • Infectious Diseases

Background:

  • Mycotic keratitis is a serious fungal infection of the cornea.
  • Exserohilum rostratum is a rare cause of fungal keratitis, typically associated with plant material exposure.
  • Hansens disease (leprosy) can predispose individuals to ocular complications.

Observation:

  • A 42-year-old male patient with a history of Hansens disease presented with symptoms suggestive of keratitis.
  • Ocular examination revealed corneal involvement consistent with a fungal infection.

Findings:

  • Microscopic and culture analysis identified an atypical species of Exserohilum rostratum as the causative agent.
  • The patient's underlying Hansens disease may have contributed to the susceptibility to this fungal infection.

Related Experiment Videos

Implications:

  • This case expands the known spectrum of fungal pathogens causing mycotic keratitis.
  • Clinicians should consider atypical Exserohilum species in the differential diagnosis of fungal keratitis, especially in immunocompromised patients or those with specific comorbidities like Hansens disease.
  • Further research may be needed to understand the specific virulence factors of this atypical strain.
