Background:

Purpose of the Study:

  • To investigate if optimizing the atrioventricular (AV) sequence using dual pacemaker stimulation can increase cardiac output (CO) in patients with advanced DCM.
  • To assess the impact of programmed AV intervals on CO and hemodynamic pressures.

  • 22 patients with advanced heart failure (NYHA class I-IV) underwent temporary dual-chamber pacing (VDD or DDD mode).

  • Pulmonary capillary and arterial pressures remained unchanged.

Conclusions:

  • Optimizing the AV sequence through pacemaker intervention can effectively improve cardiac output in selected advanced heart failure patients.
  • This approach is particularly beneficial for patients with coexisting atrioventricular and/or intraventricular conduction delays.
  • Pacemaker optimization serves as a valuable complementary treatment strategy for advanced heart failure.
