Summary

Lung volume reduction surgery (LVRS) combined with cardiac interventions improves pulmonary function and exercise capacity in emphysema patients. This combined approach is safe and effective, even enabling valve replacement in select cases.

Background:

  • Lung volume reduction surgery (LVRS) is a treatment for severe emphysema.
  • Patients with emphysema often have co-existing cardiac conditions requiring intervention.
  • The safety and efficacy of combined LVRS and cardiac procedures are not well-established.

Observation:

  • This study evaluated 12 patients with severe emphysema undergoing LVRS combined with cardiac interventions.
  • Nine patients had coronary artery disease interventions (PTCA, stent, CABG).
  • Three patients underwent valve replacement (aortic or mitral) concurrently with LVRS.

Findings:

  • LVRS combined with coronary artery disease intervention significantly improved FEV1, reduced hyperinflation, and enhanced exercise capacity (12-min walking distance) and dyspnea.
