Summary
Primary brainstem injury after head trauma is rare but often survivable. Despite initial concerns, these injuries can lead to good recovery and independence, challenging previous notions of poor prognosis.

Background:
- Primary brainstem injury from head trauma is infrequent.
- Often associated with supratentorial injuries, isolated brainstem damage is exceptionally rare.
- Shearing forces or tentorial edge impact can cause primary brainstem injury.
Observation:
- Patients may present with altered sensorium due to concurrent supratentorial injury.
- Isolated brainstem injury symptoms include internuclear ophthalmoplegia, anisocoria, rigidity, and cerebellar tremor.
- Improved imaging techniques enhance the diagnosis of these injuries.
Findings:
- Nine cases of primary brainstem injury from road traffic accidents were treated.
- Eight out of nine patients achieved independence post-injury.
- Primary brainstem injuries do not invariably lead to poor prognosis or mortality.
