    Summary

    Replacing dextrose (D5W) with crystalline amino acids (A3W) in postoperative fluid management for bladder cancer patients improved outcomes. A3W reduced complications and enhanced immune function compared to D5W in this patient group.

    Background:

    • Invasive bladder cancer and its treatment (cystectomy, radiation) cause significant nutritional depletion.
    • Preoperative malnutrition is prevalent in these patients (39%).

    Purpose of the Study:

    • To evaluate the efficacy of crystalline amino acids (A3W) versus dextrose (D5W) in postoperative fluid management for radical cystectomy patients.
    • To assess the impact on nutritional status, complications, and immune function.

    Main Methods:

    • Prospective study of 44 patients undergoing radical cystectomy and urinary diversion.
    • Comparison of postoperative fluid management using dextrose (D5W) versus crystalline amino acids (A3W).
    • Assessment of nutritional parameters, complication rates, albumin levels, and cell-mediated immunity.

    Main Results:

    • Lower postoperative complication rate in the A3W group (17%) compared to the D5W group (33%).
    • Reduced need for exogenous albumin to maintain serum levels in the A3W group.
    • Significantly higher maintenance of cell-mediated immunity in the A3W group (94%) versus the D5W group (56%).
    • No patients in the A3W group required conversion to intravenous hyperalimentation (IVH), unlike four in the D5W group.

    Conclusions:

    • Crystalline amino acids (A3W) are indicated as a superior alternative to dextrose (D5W) for hypocaloric postoperative fluid management in patients with invasive bladder cancer.
    • Early nutritional support with A3W can mitigate nutritional depletion and improve clinical outcomes in this high-risk patient population.
