Background:

Benign prostatic hypertrophy (BPH) is a common condition in aging men.
Giant prostatic hypertrophy is defined as a prostate weighing over 200 grams.
This condition can lead to significant lower urinary tract symptoms, including retention.

Observation:

An 81-year-old male presented with acute urinary retention.
The patient underwent a retropubic prostatectomy for management of his condition.
The resected prostate specimen weighed 267g, confirming giant prostatic hypertrophy.

Findings:

Pathological examination confirmed benign hyperplasia of the prostate.
The surgical procedure involved significant blood loss (1,850 ml) and autologous blood transfusion (800 ml).
A review of Japanese literature identified 32 similar cases of giant prostatic hypertrophy.
