Appendicitis is a place for clinical judgement

Diagnosing acute appendicitis remains challenging despite advances in medical imaging and clinical guidelines. Existing literature highlights variability in diagnostic accuracy, with negative appendectomies reported in 9% to 40% of cases. Prior research has shown that clinical assessment alone can lead to diagnostic uncertainty. No prior work had resolved the specific factors contributing to complicated appendicitis. That uncertainty drove this study's focus on clinical performance and associated variables. It was already known that delays in seeking care influence outcomes in acute conditions. However, the relationship between consultation delay and appendicitis severity had not been clearly established. This gap motivated an analysis of preoperative clinical indicators and their association with disease complexity. The study aimed to clarify how clinical judgment alone might predict complicated appendicitis.

Clinical data alone showed acceptable accuracy in predicting disease severity. No other demographic or clinical factor reached statistical significance. The association between consultation delay and complicated disease remained strong. These findings suggest clinical judgment can reliably identify disease complexity.
