We describe two women with overlapping symptoms of primary biliary cirrhosis and autoimmune hepatitis. Treatment with prednisolone rapidly normalized liver tests, suggesting this overlap syndrome is responsive to therapy.

Area of Science:

  • Hepatology
  • Autoimmune Diseases
  • Gastroenterology

Background:

  • Primary biliary cirrhosis (PBC) and autoimmune hepatitis (AIH) are distinct chronic liver diseases.
  • Overlap syndrome, also termed autoimmune cholangitis, presents with features of both PBC and AIH.

Observation:

  • Two female patients in their fifties presented with overlapping clinical and biochemical signs of PBC and AIH.
  • Liver biopsy confirmed cirrhosis with significant bile duct lesions.

Findings:

  • Both patients exhibited cholestasis and responded rapidly to prednisolone treatment.
  • Aminotransferase levels normalized quickly following prednisolone administration.

Implications:

  • This case series highlights the recognition and management of the PBC/AIH overlap syndrome.
