This study found no significant difference in complications between Carbomedics and St. Jude Medical bileaflet mechanical prostheses for mitral valve replacement. Patient outcomes were similar, regardless of the prosthesis type used.

Area of Science:

  • Cardiovascular Surgery
  • Biomaterials Science
  • Clinical Outcomes Research

Background:

  • Bileaflet mechanical heart valves are crucial for mitral valve replacement (MVR).
  • Carbomedics (CM) and St. Jude Medical (SJM) are common prostheses.
  • Evaluating their comparative safety and efficacy is essential.

Purpose of the Study:

  • To evaluate thromboembolic and hemorrhagic complications associated with CM and SJM prostheses.
  • To identify predictive risk factors for adverse events after MVR.
  • To compare the performance of CM versus SJM bileaflet mechanical prostheses.

Main Methods:

  • A prospective observational study of 625 patients undergoing MVR between 1989 and 1994.
  • Included patients receiving either CM (n=240) or SJM (n=385) prostheses.

Related Experiment Videos

  • Data collected on pre-operative variables, thromboembolism, thrombosis, and hemorrhage.
  • Main Results:

    • No significant difference in overall thromboembolism (5.0%/patient-year) or hemorrhage between CM and SJM prostheses.
    • Pre-operative variables did not predict adverse events.
    • Most thromboembolic events occurred with inadequate anticoagulation (INR < 2.5).

    Conclusions:

    • The Carbomedics and St. Jude Medical bileaflet mechanical prostheses demonstrated comparable performance in this single-institution experience.
    • No significant differentiation in thromboembolic or hemorrhagic complications was observed.
    • Further comparative evaluations may be warranted.
