Background:

  • Scleritis, an inflammation of the sclera, can lead to vision loss.
  • Ocular infections can sometimes present with scleral involvement.

Observation:

  • A retrospective case series identified three patients with scleritis linked to Haemophilus influenzae.
  • Two patients had no history of collagen vascular diseases; one had arthritis with positive autoimmune markers.
  • All patients had undergone ocular surgery (cataract or strabismus) more than six months prior to scleritis onset.

Findings:

  • Nodular abscesses with scleral necrosis were observed in all affected eyes.
  • Cultures from these abscesses consistently revealed Haemophilus influenzae.
  • Treatment involved topical, subconjunctival, and systemic antibiotics, leading to resolution of inflammation and improved visual acuity.

  • Haemophilus influenzae should be considered in the differential diagnosis of infectious scleritis, especially post-ocular surgery.
  • Early and accurate diagnosis and appropriate antimicrobial therapy are vital for successful management and visual preservation.
