Background:

  • The lumbar plexus is a crucial network of nerves in the lower back, originating from the anterior rami of the lumbar spinal nerves.
  • Variations in its formation can impact surgical procedures and understanding of neurological conditions.

Observation:

  • A complex bilateral anatomical variation in the lumbar plexus was observed in a 32-year-old male cadaver.
  • The left lumbar plexus was postfixed, situated posterior to the psoas major muscle.
  • The right lumbar plexus was prefixed, located within the substance of the psoas major muscle.

Findings:

  • Significant differences in nerve formation were noted on each side.
  • On the left, the femoral nerve arose from lumbar nerves L2-L5.
  • On the right, the lateral cutaneous nerve of the thigh originated from L1-L2, the femoral nerve from L1-L3 and L5, and the obturator nerve from L1-L3.

Implications:

  • These findings underscore the extensive variability in human lumbar plexus anatomy.
  • Such variations are critical for anatomists, surgeons, and clinicians to consider during diagnosis and treatment.
  • This case report contributes novel data to the existing literature on lumbar plexus anomalies.
