Background:

Older adults with cognitive impairment often exhibit challenging behaviors.
Day care services aim to provide support and manage behavioral issues in this population.
National Health Service (NHS) day hospitals and non-NHS day care facilities offer distinct service models.

Purpose of the Study:

To compare behavioral disturbances in confused older adults attending NHS day hospitals versus non-NHS day care.
To explore underlying differences in service provision and referral patterns.

Main Methods:

A behavioral rating scale assessed 20 NHS day hospital attendees and 64 confused attendees at seven non-NHS day care facilities.
Referral information was collected for all participants.
The study was conducted in one UK health authority.

Main Results:

NHS day hospital attendees exhibited slightly higher levels of restlessness, friction, sexual disinhibition, poorer memory, and reduced mobility.
Qualitative analysis revealed significant differences in operational focus: NHS day hospitals prioritized assessment and throughput, while non-NHS facilities emphasized social support.

Conclusions:

While minor behavioral differences exist, they do not fully capture the distinct roles of NHS day hospitals and non-NHS day care.
Referral reasons and service objectives are critical factors differentiating these facilities.
NHS day hospitals serve a unique purpose beyond simply filling a gap in care.
