Summary

Laparoscopic cholecystectomy significantly reduces diaphragmatic activity, as measured by direct EMGdia and indirect indices. Diaphragmatic function shows partial recovery 24 hours after the procedure.

Background:

Purpose of the Study:

  • To directly evaluate diaphragmatic activity following laparoscopic cholecystectomy.
  • To compare direct EMGdia findings with indirect indices of diaphragmatic function.
  • To assess the temporal recovery of diaphragmatic activity post-surgery.

Main Methods:

  • Thirteen patients undergoing laparoscopic cholecystectomy were studied.
  • Diaphragmatic EMG (EMGdia) was recorded along with gastric and esophageal pressures, and ribcage/abdominal cross-section areas.

    • Laparoscopic cholecystectomy led to a significant reduction in diaphragmatic activity, evidenced by decreased deltaPgas-insp, deltaPdi-insp, and Vabd.
    • Transdiaphragmatic pressure (deltaPdi-insp) decreased significantly at 1 and 6 hours postoperatively (P < 0.05).
    • Abdominal cross-section area (Vabd) also showed a significant decrease at 1 and 6 hours (P < 0.05), with partial recovery by 24 hours.

    Conclusions:

    • Direct and indirect measurements confirm reduced diaphragmatic activity after laparoscopic cholecystectomy.
    • Diaphragmatic function begins to recover by 24 hours post-surgery.
    • EMGdia provides a reliable method for assessing diaphragmatic function in the postoperative period.
