  • Entamoeba histolytica is often considered nonpathogenic in homosexual populations in Western countries.
  • Invasive amebiasis typically presents with gastrointestinal symptoms.

Observation:

  • A case of amebic colitis in a 75-year-old Japanese homosexual man is presented.
  • The patient had an isolated cecal ulcer but no gastrointestinal complaints.
  • This challenges the assumption of nonpathogenicity in this demographic.

Findings:

  • Absence of gastrointestinal symptoms does not exclude invasive amebiasis.
  • Pathogenic Entamoeba histolytica can cause disease even in asymptomatic individuals.
  • Early differentiation of pathogenic from nonpathogenic Entamoeba species is vital.
