Summary

Eclampsia, a severe pregnancy complication causing seizures, continues to cause maternal deaths. Improved training and standardized protocols for managing severe pre-eclampsia are crucial to prevent seizures and reduce mortality.

Area of Science:

  • Obstetrics and Gynecology
  • Maternal Health
  • Neurology

Background:

  • Eclampsia, characterized by seizures in pre-eclamptic women, is a significant contributor to maternal mortality and morbidity.
  • Despite its long history, management practices for eclampsia remain inconsistent.
  • A concerning number of eclamptic seizures occur after hospital admission, indicating potential failures in identification or treatment.

Purpose of the Study:

  • To highlight the persistent challenges in managing eclampsia.
  • To emphasize the need for improved diagnostic and therapeutic strategies for severe pre-eclampsia and eclampsia.
  • To advocate for standardized management protocols to reduce substandard care.

Main Methods:

  • Review of existing literature and clinical guidelines on pre-eclampsia and eclampsia management.
  • Analysis of trends in maternal mortality and morbidity related to eclampsia.
  • Emphasis on the importance of education and training for healthcare professionals.

Main Results:

  • Eclampsia remains a critical issue in maternal healthcare.
  • Current management often fails to prevent seizures, even in hospital settings.
  • Substandard care elements in eclampsia management are repeatedly identified in maternal death reports.

Conclusions:

  • Enhanced education and training for obstetricians, anaesthetists, midwives, and GPs are essential.
  • Development and consistent implementation of management protocols for fluid balance, antihypertensives, and anticonvulsants are vital.
  • Universal adoption of evidence-based guidelines can significantly decrease substandard care and improve maternal outcomes.
