Area of Science:

  • Dermatology
  • Parasitology
  • Microbiology

Background:

  • Demodicosis is an infestation of Demodex mites, commonly found on human skin.
  • While often asymptomatic, Demodex mites can cause various skin lesions, particularly in immunocompromised individuals or with specific mite strains.
  • Unusual presentations of demodicosis can mimic other dermatological conditions, complicating diagnosis.

Observation:

  • A 38-year-old female presented with a sudden onset of tuberous, pustular lesions on her chin.
  • Clinical examination, histology, and microscopy of pustule smears were performed.
  • Histopathology revealed follicular cysts and chronic granulomatous perifolliculitis with Demodex folliculorum.

Findings:

  • The diagnosis of demodicosis was confirmed by identifying a high number of Demodex folliculorum mites.
  • No underlying immunological defects, tumors, or systemic disorders were detected in the patient.
  • The patient's lesions showed rapid improvement following treatment.

Implications:

  • This case highlights demodicosis as a potential cause of unusual pustular facial tumors.
  • Early and accurate diagnosis through integrated clinical, histological, and microbiological evaluation is crucial.
  • Combined therapy with corticosteroids and metronidazole appears effective for managing severe demodicosis presentations.
