Summary

Women with high-risk chronic hypertension benefit from antihypertensive therapy and close management to improve pregnancy outcomes. Low-risk chronic hypertension and prophylactic acetylsalicylic acid do not improve maternal or fetal results.

Aspirin for the prevention of placenta-mediated complications in pregnant women with chronic hypertension.

Journal of gynecology obstetrics and human reproduction·2020

Area of Science:

  • Obstetrics and Gynecology
  • Cardiology
  • Nephrology

Background:

  • Chronic hypertension in pregnancy increases risks for superimposed pre-eclampsia, abruptio placentae, fetal growth retardation, and prematurity.
  • High-risk chronic hypertension, defined by severe hypertension or pre-existing cardiovascular/renal diseases, further elevates complication frequencies.
  • Target organ damage in chronic hypertension also correlates with increased adverse pregnancy outcomes.

Purpose of the Study:

  • To evaluate the impact of antihypertensive therapy and prophylactic low-dose acetylsalicylic acid on pregnancy outcomes in women with chronic hypertension.
  • To differentiate management strategies based on risk stratification (high-risk vs. low-risk chronic hypertension).

Main Methods:

  • Review of existing literature and clinical guidelines on managing chronic hypertension during pregnancy.
  • Analysis of complication rates and treatment efficacy in different risk groups.

Main Results:

  • Antihypertensive therapy and close management are recommended for high-risk chronic hypertension to improve maternal and fetal outcomes.
  • Antihypertensive treatments do not improve pregnancy outcomes in women with low-risk chronic hypertension.
  • Prophylactic low-dose acetylsalicylic acid does not reduce superimposed pre-eclampsia or improve perinatal outcomes in these pregnancies.

Conclusions:

  • Risk stratification is crucial for managing chronic hypertension in pregnancy.
  • Targeted antihypertensive therapy and monitoring are essential for high-risk pregnancies.
  • Low-risk chronic hypertension and prophylactic aspirin do not warrant intervention for improved pregnancy outcomes.
