Summary

Multiple nodular pulmonary tuberculosis can mimic lung cancer metastases. This study highlights the importance of considering tuberculosis in differential diagnoses for patients with multiple lung nodules.

Background:

  • Metastatic lung cancer is often suspected with multiple pulmonary nodules.
  • Pulmonary tuberculosis (TB) can present with similar imaging findings.
  • Early diagnosis and appropriate treatment are crucial for TB management.

Observation:

  • Four asymptomatic patients presented with multiple (15-50) spherical nodules (3-10 mm) with sharp margins.
  • Diagnostic imaging revealed lesions suggestive of hematogenous dissemination.
  • Skin tests (Mantoux) showed positive reactions in 2 patients.

Findings:

  • Histopathology confirmed epithelioid cell granulomas in all patients.
  • Diagnosis of pulmonary tuberculosis was established despite negative acid-fast bacilli.
