Summary

Intrathyroid parathyroid adenoma is a rare cause of hyperparathyroidism. Consider ectopic glands, including those within the thyroid, when standard surgery for primary hyperparathyroidism fails to locate a pathological gland.

Area of Science:

  • Endocrinology
  • Surgical Pathology

Background:

  • Primary hyperparathyroidism is often caused by parathyroid adenomas.
  • Surgical excision is the primary treatment for symptomatic hyperparathyroidism.

Observation:

  • Two cases of intrathyroid parathyroid adenoma, a rare cause of hyperparathyroidism, are presented.
  • In one case, an intrathyroid nodule was diagnosed as parathyroid adenoma post-excision.
  • In the second case, preoperative imaging suggested an intrathyroid parathyroid adenoma.

Findings:

  • Intrathyroid parathyroid adenomas can present as a cause of primary hyperparathyroidism.
  • These rare adenomas may be missed during standard surgical exploration.

Implications:

  • Consideration of ectopic parathyroid glands, specifically intrathyroid adenomas, is crucial when a pathological gland is not found during surgery for primary hyperparathyroidism.
