Summary

The haptoglobin (Hp) 2-2 phenotype increases peripheral arterial occlusive disease (PAOD) risk but is linked to better walking distance. This may be due to the Hp 2-2 molecule's angiogenic properties.

Background:

  • Haptoglobin (Hp) 2-2 phenotype is a known genetic risk factor for coronary atherosclerosis.
  • Peripheral arterial occlusive disease (PAOD) is a manifestation of atherosclerosis affecting limb arteries.

Purpose of the Study:

  • To investigate the association between haptoglobin phenotypes and peripheral arterial occlusive disease (PAOD).
  • To explore the clinical implications of the Hp 2-2 phenotype in PAOD patients.

Main Methods:

  • Haptoglobin phenotypes were determined in 141 PAOD patients and compared to a reference population (n=1000).
  • Serum lipids, inflammatory markers, blood pressure, and vitamin C levels were analyzed across Hp phenotypes.
  • Maximal walking distance was assessed in PAOD patients with severe atherosclerotic lesions.

Main Results:

  • PAOD patients showed a decreased Hp1 allele frequency and overrepresentation of the Hp 2-2 phenotype (OR 1.82, P < 0.001).
  • This overrepresentation was more pronounced in non-diabetic and non-smoking PAOD patients.
  • Hp 2-2 patients had lower serum vitamin C levels (P < 0.05) but exhibited a longer maximal walking distance (P < 0.05) in severe PAOD.
  • Serum lipids, inflammatory parameters, and blood pressure were comparable among Hp phenotypes.

Conclusions:

  • The Hp 2-2 phenotype is associated with an increased risk of developing PAOD.
  • Despite increased risk, the Hp 2-2 phenotype correlates with improved maximal walking distance in severe PAOD.
  • The enhanced walking capacity in Hp 2-2 individuals may be linked to the angiogenic properties of the Hp 2-2 molecule.
