Summary

High-dose chemotherapy (HDC) with stem cell support shows promise for treating solid tumors by overcoming drug resistance. However, more randomized trials are needed to define its role in most solid tumor cancers.

Background:

Advances in treating hematological malignancies have led to exploring high-dose chemotherapy (HDC) with stem cell support for solid tumors.
Drug resistance is a primary cause of treatment failure in solid tumors, making HDC a logical strategy to investigate.

Observation:

Laboratory data indicate a dose-response relationship, where higher chemotherapy doses can overcome resistance.
Clinical trials in solid tumors demonstrate a clear dose-response effect for chemotherapy.

Findings:

Numerous studies have investigated HDC in solid tumors, especially those initially chemo-sensitive but prone to relapse.
Except for breast cancer, randomized trials are lacking to establish the role of HDC in other solid tumors.
Many existing trials are small, pilot studies in pretreated patients with advanced disease, requiring cautious interpretation.

Implications:

HDC is a potential strategy to overcome drug resistance in solid tumors.
Further rigorous, randomized clinical trials are essential to define the therapeutic role and efficacy of HDC in various solid tumors.
